Over the last 5 years leadership has discussed merging with another smaller hospital in the local area. This hospital is a 120-bed facility that has faced various financial and patient care safety issues. As a result of the merger the management structure has been realigned to ensure operational functions of both physical facilities are controlled by a single manager-you have been chosen as that new manager.

Clearly describe why you chose this function and why you feel this particular function is essential to your effective management as a result of the merger. Process Mapping: Students are required to research an existing health care organization of their choice. In your research you should identify a quality improvement (QI) initiative that will improve the way patient care is delivered at the facility. The QI can be the result of organizational need, accreditation standards, and/or regulatory requirements.
Instructions: Clearly state the specific QI objective. Provide a brief description articulating why the QI is important and how the quality of patient care will be improved as a result of the QI. In addition, identify an executive, team lead, and staff member as the stakeholders who are responsible for the implementation of the QI and explain what roles they play in the implementation of the objective. It will be necessary to generate a flow chart that specifically emphasizes the steps necessary for implementing the QI as well as the position that is most appropriate for performing the tasks and exact duties of the step.

Identify a health technology or a specific aspect of a payment system that is changing for your health care setting. Work as a team to prepare a PowerPoint presentation to educate and inform your co-workers about the recent change. You must identify the health setting, and clearly identify the source of the change and the ramifications of the change on the setting. Suggest how the organization and managers in the organization could use this change as an opportunity to improve the organization. Explain how. No more than 12 slides. Include a title slide with names of contributors and a reference slide at the end. You may use the notes section of the slide.
Question 4
The class examined the Joint Commission’s framework for root cause analysis of sentinel Incidents in health care organizations. For purposes of this project, a critical incident is a key occurrence, but it is not a sentinel event. A bow-tie analysis places the critical incident at the center of the framework and identifies in graphic format the root cause leading up to the incident on the left side of the critical incident. On the right side of the incident, key preventive measures are graphically represented. In one image, you will capture the incident (at the center), the causes for failure in quality, and (on the right) preventive measures a manager can put in place to stop this failure from happening in the future. The final graphic will appear as a bow-tie. The instructor will provide an example. Each student will select a critical incident for approval by the instructor.
